Your story matters. Your identity can be safe. This is a confidential pre-interview form to help us understand the context of your experience. Names can be changed, and you can wear a mask during the interview (unless you say otherwise). Share only what you feel comfortable with. Nothing here is shared without your consent.
Alias or Code Name: What name should we use for you on the podcast? (You can use a nickname — ex: Crypto Widow, Case #17, The Silent Intern, etc. Make it easy to say)
2. Story Summary: In 1–3 sentences, what is your story about?
3. Who Was Involved? Do you want to use titles, not real names, if so please list (ex: 'ex-boyfriend,' 'manager,' 'former friend'). Only list name if you feel comfortable with this. Who else played a role in this story?
